The use of CT technology has been a part of my diagnostic workup for the past 16 years. However, in the past, the transfer of prosthetically directed information to perform implant surgery has lacked precision and guidance. Despite the use of CT information, surgical execution remained largely intuitive not guided.
In June of 2002 I experienced a paradigm shift which has been remarkable and exciting. Today, implant placement can be prosthetically directed. Through the use of special scanning appliances it is now possible to transfer prosthetic outcomes to the CT study, develop a computer generated treatment plan and through the use of rapid prototyping have a surgical guide built to facilitate the outcome.
Because there are many surgical guide options to select from, I would recommend the incorporating a guide type which most closely reflects your current philosophy of implant placement. This would most likely involve the use a bone-supported surgical guide. The use of a bone-supported surgical guide would enable the surgeon to compare both the old and new way of implant placement. Develop trust and confidence in the use of SimPlant®. Like any type of radiograph the doctor will need to master the learning curve to understand its clinical relevance and subtleties. Become comfortable with the development of a prosthetically directed treatment plan and its surgical implementation. After gaining confidence move to a tooth supported and eventually to a mucosal supported guide. By incrementally incorporating the various guide strategies, you will avoid setbacks, improve your understanding and interpretation of CT information and eventually place implants without bone exposure.
I have assembled a series of computer guides treatment plans and cases which reflect my experiences since June of 2002. On a personal note, I would like to thank Dr. Philippe Tardieu for his guidance and encouragement. These cases, in large part, reflect his influence, creativity and enthusiasm for this paradigm shift in implant treatment.
